Principles of Incident Investigations

Incident investigation goes beyond simply responding to accidents—it involves systematically exploring how and why events occurred, so that underlying causes and system weaknesses can be identified and addressed. The Principles of Incident Investigation course provides a comprehensive overview of the processes and practical skills needed to examine workplace incidents effectively.

A thorough investigation prioritises learning and prevention, rather than blame. By collecting evidence, analysing contributing factors, and making defensible recommendations, investigations drive meaningful improvements to workplace safety and organisational culture. Investigators consider everything from procedures and supervision to equipment, work environment, and human factors, helping ensure hazards are managed before they cause further harm.

This course is designed to equip OHS practitioners, managers, and investigation team members with the frameworks, legal context, and step-by-step techniques for planning, conducting, and reporting on incidents. Participants will learn how to preserve evidence, interview witnesses, use causal analysis tools, and implement improvements that strengthen safety systems and build trust across the organisation.
